Etc. The Clerk Of Court Shall Furnish Thomas With A Blank Copy Of This Courts Current Standard Prisoner In Forma Pauperis Form Bearing The Above Civil Action Number.2 Thomas May Use This Form To Seek Leave To Proceed In Forma Pauperis If He Cannot Afford To Pay The Fees To Commence This Case. Thomas Is Directed To Complete And Return The Declaration Form Attached To This Order, With His Original Signature, To The Court Within Thirty (30) Days Of The Date Of This Order To Cure This Deficiency With The Complaint. The Clerk Of Court Is Directed To Send A Copy Of This Order To Thomas Along With A Copy Of The Attached Declaration Form And The Courts Current Standard In Forma Pauperis Form Bearing The Above Civil Action Number And Note The Mailing On The Docket.
